At the Darmstadt-Dieburg nursing centre, you will personally advise on all questions relating to care, with particular emphasis on care advice according to § 7a SGB XI. In addition, you take over the holistic advice on supply management issues and thus contribute to a demand-oriented supply. Complex facts and existing supply deficits identify you early, process these solution-oriented and develop individual supply plans. You have completed training in health care and ideally bring the qualification to the nursing counselor/care counselor according to § 7a SGB XI. If this is not yet available, bring the willingness to acquire the qualification accordingly. You were able to gain first practical experience in nursing, especially in cooperation with nursing services and in outpatient and inpatient care. In a comparable activity, you have acquired expertise in the provision of statutory health insurance and care insurance as well as in case management. Alternatively, you will bring well-founded experience in case management of performance-related issues within the statutory health and care insurance. You are versed in assessing current therapy and supply plans and identifying potential supply deficits at an early stage. With a view to sustainability and quality of supply, you can ensure a demand-oriented, efficient and long-term optimised supply.
