The position includes tasks in the field of sociotherapy according to § 37a SGB V as well as the flexible support in the outpatient care acc. §§78, 113 SGB IX. Objectives of sociotherapy: The aim of the societal therapist is to help patients avoid future hospital treatments, to replace or shorten them with the help of sociotherapy or to enable necessary hospital treatments, as well as the coordination of prescribed services and guidance and motivation necessary in individual cases.
