Your tasks:

• Accompanied with heart: You support people with chronic mental impairments or addiction diseases in Witten and the surrounding area and accompany them in their everyday life
• Individual planning: You develop and develop personal participation plans together with clients to promote their self-employment
• Support in everyday life: You help in administrative matters, coordinate existential measures and are reliable contact persons in crises or difficult situations
