Your tasks:

• Target group: accompaniment of adult people with chronic-psychic diseases in everyday life
• Participation: Support for the development, formulation and implementation of individual goals and the establishment of a self-determined and independent life management as self-determined as possible
• Case Responsibility: Implementation and further development of the jointly developed participation plans in cooperation with various stakeholders
• Specialist tasks: independent implementation of basic services
• Exchange of knowledge: transfer of professional knowledge and experience to colleagues and trainees and interns
• Network work: Close cooperation with internal and external partners in the social area such as members, health and social institutions, administration or church communities
• Documentation: Careful documentation and active participation in handovers and team discussions

What you should bring:

• Qualification: Completed three-year training, for example, as a curative nurse, educator, ergotherapeut or also care professionals (m/w/d)
• Experience: Ideally experience in supporting people with chronic-psychic diseases
• Communication: Joy in dealing with different people as well as an independent and team-oriented way of working
• Engagement: motivation, structured and creative to work and introduce your own ideas
• Flexibility: willingness to work in shift services with early and late services and a maximum of two weekend services per month without planned night services
• Knowledge: Basic computer skills in Word, Excel and Outlook
